Tooth decay and tooth wear: what is the difference?

Tooth tissue can be lost through decay, acid exposure and wear. These processes can overlap, so an examination is needed before deciding how to protect or restore a tooth.

Does every damaged area mean decay?

No. Decay is one cause of tissue loss, but acids and mechanical wear can also change the shape of teeth. Simply filling a damaged area may not prevent further loss if its cause remains active.

Do not diagnose a lesion only by colour or pain. Early decay may look like a white mark and cause no symptoms. A dark stain does not necessarily indicate active disease. The NIDCR tooth-decay guide explains how the process develops.

Four terms you may hear

TermWhat it describesUseful information for your dentist
Tooth decayMineral loss involving plaque bacteria and sugars.Cleaning, diet, previous cavities and fluoride use.
ErosionAcid effects separate from acids produced by plaque bacteria.Drinks, foods, reflux and vomiting.
AbrasionMechanical wear involving an external factor.Brushing pressure, abrasive products and habits.
AttritionWear from contact between teeth.Changes in shape, clenching, grinding and restoration damage.

These categories explain mechanisms, not a home diagnostic test. The location and appearance of a defect do not always identify a single cause.

Acid erosion: more than sugary drinks

Acidic foods and drinks, reflux or repeated vomiting can contribute to tissue loss. How often and how long acid contacts the teeth matters as well as the product itself. A drink can be sugar-free and still be acidic. The ADA explains dental erosion and ways to reduce exposure.

Reflux or recurrent vomiting may need medical care alongside dental treatment. Rebuilding a tooth without addressing the acid source may leave the problem active. Discuss habits and symptoms instead of removing whole groups of nutritious foods without advice.

Can brushing be too forceful?

Excessive pressure and abrasive products can contribute to wear, particularly alongside acid exposure. Gentle, effective cleaning is preferable to harder scrubbing. Ask a hygienist to demonstrate a technique suited to your brush and teeth.

Exposed dentine can become sensitive. Its microscopic tubules are not the same as a tooth’s root canals, and sensitivity does not automatically mean root canal treatment is needed.

When is tooth-to-tooth wear concerning?

Some wear develops over time. A rapid change in tooth shape, chips, pain or loss of function warrants assessment. Existing wear marks alone do not prove that significant sleep bruxism is currently occurring.

If you notice clenching or grinding, discuss it before buying a ready-made guard. Protection should be selected after an examination.

What does “abfraction” mean?

Abfraction describes a proposed role of stress in non-decay defects near the gum line. A wedge-shaped defect should not automatically be attributed only to the bite. Multiple factors may contribute, and the shape alone does not justify irreversible bite adjustment or orthodontic treatment. A British Dental Journal review discusses the limits of the evidence.

Does every defect need a filling?

The dentist considers activity, depth, progression, cleanability, symptoms and the risk of further damage. Care might involve prevention, monitoring, sensitivity management or restoration. Remineralisation can help early mineral loss, but cannot recreate a missing piece of the crown.

Bring details of sensitivity, dietary patterns, reflux and visible wear to your appointment. At ProDent in Nysa, restorative assessment can establish both the treatment need and what may help limit further damage.
